Historical Perspectives and Evolution of Chronotherapy

Abstract
This chapter is about the development and significance of aligning the principles of chronotherapy, or the synchronization of drug administration with the body\'s biological rhythms. Chronotherapy is the practice of administering drugs at times that coincide with the body\'s internal clock, or circadian rhythms, in order to increase drug effectiveness and decrease the incidence of side effects. The first part of the chapter begins with a glance at ancient forms of medicine that recognized the natural timing of the world, where the early healers understood the role of the sun, the moon, and the changes in seasons on the human being. Even though they may not have stated it in those terms, early herbal medicine, acupuncture, and Ayurveda did seem to incorporate these concepts. The modern idea of chronotherapy, however, can be said to have started in the 18th century, with the discovery of the idea of circadian rhythms, when scientific research revealed the existence of the internal biological timing of the body\'s functions. The field of chronobiology, which is the study of biological rhythms, was developed in the 20th century based on the work of scientists such as Franz Halberg. Halberg\'s work, in particular, identified the time dimension in biology and pharmacology. Since then, clinical studies have been instrumental in showing the benefits of synchronizing drug administration with the body\'s clock. From chemotherapy to antihypertensive drugs, a large number of studies have shown that drug effects are enhanced and side effects reduced when the timing of drug administration is controlled. It also discusses the current challenges faced, including patient variability and logistical barriers, in addition to current and emerging technologies such as wearable devices with the potential to further personalize chronotherapy. The chapter provides a panoramic view of the historical evolution and future directions of chronotherapy by tracing these developments.
